You have taken the most important step toward building a successful dental sleep medicine practice, and we are glad you are here.

This page walks you through the key steps in your onboarding process and gives you a clear picture of everything available to you inside your dashboard. You have flexibility in how you work through the resources here. The one firm requirement is this: your Onboarding Form and Credentialing Questionnaire must be completed first. They set everything else in motion. Once those are submitted, your dashboard is yours to explore and use in the order that fits your schedule and your practice.

Your first step is to meet with your TDSS advisor. This single conversation will save you time on every step that follows.

The Onboarding Form is the foundation of the carrier credentialing process. It captures everything our team needs to begin your setup and is organized into seven tabs.

Before You Start

  • Any PDF files, such as your DEA certificate, malpractice coverage documents, or other formal credentialing documents, must be uploaded on the Personal tab.
  • Photos and images, including JPEGs, PNGs, or photos taken directly from your phone, can be uploaded inline on any of the remaining tabs.
  • The final tab, Limited POA, is a Limited Power of Attorney that authorizes TDSS to prepare and submit your credentialing packages to contracted carriers on your behalf. This is what allows our team to do the work for you.

The Seven Tabs

  • Personal — Your name, contact information, and personal identifying details. Upload all PDF documents here.
  • Insurance — Your malpractice and liability insurance information.
  • License & Credentials — Your dental license, AADSM membership status, and any relevant specialty credentials.
  • Logins — Login credentials and access information for systems you will be connected to as a TDSS member.
  • School & Work History — Your educational background and professional work history, required for carrier credentialing applications.
  • DSM Practice Info — Details about your current practice, including location, hours, staff, and existing DSM experience.
  • Limited POA — Your authorization for TDSS to manage and submit your credentialing packages on your behalf.

Each tab in the Onboarding Form saves independently. When you have completed a tab, click SUBMIT before moving to the next one. Once all tabs are submitted, our team is notified and your credentialing process begins.

Step 02Complete Your Credentialing Questionnaire

The Credentialing Questionnaire is a standard professional disclosure form required by medical carriers as part of the credentialing process. It consists of a series of yes/no questions covering areas such as licensure history, clinical privileges, professional liability, and fitness to practice. For most providers, it takes only a few minutes to complete.

These are standard disclosures required by carriers across the healthcare industry. Every provider who credentials with a medical carrier completes a form like this one. It is not a TDSS-specific evaluation; it is simply a required step in the carrier application process.

Between the Onboarding Form and the Credentialing Questionnaire, you provide this information once and only once.

From that point forward, it is our job to get you credentialed, whether in-network or out-of-network, with every carrier TDSS is contracted with. Our team manages the entire process, from preparing your applications to tracking their status with each carrier. You focus on your practice. We handle the paperwork.

Step 03Register for the DSM Practice Certification Program

This program is built around a learn-while-doing model, designed to get you in front of patients quickly while building your clinical and practice management skills in parallel. The curriculum is scalable and flexible, structured to be customized to the needs and philosophy of your individual practice rather than a one-size-fits-all approach.

The program spans approximately 28 weeks and is organized into 12 structured sessions covering areas that include practice assessment, sleep apnea screening, referral and healthcare navigation, EMR onboarding, case presentation, marketing, community outreach, clinical review, financial reporting, and advanced practice development. Participants earn 18 or more PACE-accredited CE hours upon completion.

Enrollment capacity is limited. Once your forms are submitted, we encourage you to register promptly to secure your spot.

Please note: enrollment in the DSM Practice Certification Program requires that both your Onboarding Form and Credentialing Questionnaire be fully submitted before you register. Do not register until both are complete.

Step 04Schedule Your Advanced Clinical Training

Advanced Clinical Training is where your clinical competency is built and validated. The centerpiece is the Two-Day Advanced Dental Sleep Medicine Seminar and Workshop, a 16-credit PACE-accredited program that covers the full clinical scope of dental sleep medicine: patient selection and screening, appliance selection and titration, interpretation of sleep study results, medical-dental collaboration, and management of complex cases. This seminar is led by clinicians with over 40 years of combined experience in the field and is one of the most comprehensive hands-on DSM training programs available anywhere in the country.

The course is held at the office of Dr. Martin Denbar in Austin, Texas and is designed for both the provider and a dental assistant to attend together. It runs across two consecutive days, Thursday and Friday, and is scheduled on demand based on a time that works for you and Dr. Denbar. There is no fixed calendar; the course is arranged around your availability.

As a full TDSS member, this course is included in your membership at no additional charge for both you and your assistant.
